Family Get-In
You can check your loved one’s information and control their medical services online if you’re a parent or carer and add them to your MyChart account. Three forms of “proxy access” are available: one for legal guardianships, one for youths between the ages of 13 and 17, and one for minors under the age of 13. Fill out a Proxy Form for consent at your provider’s office – download the relevant form below to seek access. Make sure you submit everything to the given address and include any supporting paperwork.
We need to submit a fresh proxy request each time your guardian or child moves up a tier.

Any Luminis Health healthcare professional can help you enrol in MyChart. Just go online or in person to Luminis Health Medical the Group, Luminis Health Imaging, or others participating practise and receive a MyChart activation number. You can create one’s own user password and user name by logging in using your activation code.
After being released from the medical facility, emergency room, or outpatient clinic, if you haven’t already enrolled, you can create a profile by calling the MyChart Support that 833-254-3600.
